AXIS Enrich Vendor Application FY2022-23

AXIS International Academy is excited to offer after-school enrichment opportunities to our families!

Interested vendors, please take note of the information and complete the application below:

All staff leading Enrich activities will be required to pass a background check (including volunteers). Background check fees will be paid by the vendor.
It is the responsibility of the vendor to collect Assumption of Risk Waiver for all participating students. Waivers are due before your first class.
All vendors must provide Proof of Insurance (with AXIS International Academy listed as additional insured)
Each organization must have at least one staff member certified in First Aid and CPR at all times, during each class.
Clear communication is something we highly value at AXIS. Please provide a swift response to families as they register, make payment, and communicate directly with your organization (AXIS will not liaise payment or registration).
Parent pick-up process: Each student must be signed out by the individual who registered the child, or a designated individual they listed in their registration packet (please include information about pick-up policies in the information packet you give registered families.) For the safety of all children, it's important that your staff is very organized with this process. The vendor must stay with children until they have been picked up.
AXIS will gather feedback from families and vendors at the end of each session, and we may change programming options offered or how we organize this process based on this feedback. You are absolutely welcome (and encouraged) to gather your own feedback from families, as well.
AXIS is moving away from combined age classes. Please consider elementary-only, upper- vs. lower-grade, or new vs. alumni student options when planning your schedule.
Our new location has limited options for after-school spaces. Because we are using many shared spaces, we are limited to classrooms and our fenced playground at this time. If you have questions/concerns regarding spaces, please email [email protected].
