AXIS Return to School Form

Hello AXIS Preschool and K-5 Families, 

As we enter our final planning phase for the upcoming school year, it is critical we know your preference at this time on returning to school. Before beginning this form we encourage you to review the information highlighted in the fall update that accompanied this form. We understand your choice may change between now and mid-August, and if that happens, please complete this same form again with your updated preferences. There are four brief sections:

A. Basic Information,

Because of the importance of face coverings in reducing the spread of COVID-19, AXIS will be having staff and children wear masks. It will not be a one-size-fits-all requirement, but will be age-appropriate and at times when mask wearing is most essential (during transitions, entering the building, unable to distance, etc.). Does your child have a medical condition that prevents them from being able to wear a mask?

D. Before School Care, After School Care, and School's Out Care - AXIS Educare for Preschool and K-5 Families

Please complete the information regarding before school care and after school care below, as this information will be included in determining your drop off and pick up time. To ensure safe physical distancing during these times, families will be assigned a pick up and drop off time slot/window.

AXIS is licensed to offer childcare to children ages 3 - 12 years old. This includes before care and after care and care when school is not in session. This license may allow us to offer child care even if the K-5 is forced to close due to public health or executive orders. AXIS Educare operates like childcare with more integration of education, learning and enrichment, offering daily care from 6:30am-5:30pm with rates comparable to other childcare options. We are looking at options for integrating AXIS Enrich into the AXIS Educare after school option, with AXIS staff providing the enrichment (not external organizations). If the K-5 is ordered to close and AXIS Educare can operate for K-5 children, we plan to offer remote learning  onsite with staff tutors.

We do not have an update on AXIS bussing at this time, and we will email families as soon as we have that information. If you did not ride the bus last year, and absolutely need the bus to get to AXIS please email Angie at [email protected].
