DETAILS ON CURBSIDE COVID-19 TESTING

As a service to our AXIS families, we are offering curbside COVID-19 testing. This is open to any student who is required to test per the Return to Learn flowchart, or any student who would like to test following an illness in the household. We encourange all families to take advanage of this service anytime someone in the household has been experiencing symptoms of COVID-19.

When you arrive for your appointment, please pull up to the curb in front of the school, and then call the school to let us know you have arrived. A staff member will come to meet you at the car to administer the nasal swab. You will receive a follow up phone call by 5pm only if the result is postivie. If you do not hear from us, you can assume the results are negative and your child can return to school as instructed.

About the Test:

This program uses Abbott Laboratories BinaxNOW tests provided by the federal government. We will only test with your consent. If you are willing to provide consent for us to administer this test on your child, please fill out this form.

What is the test? If your child is symptomatic or part of a group that is designated for testing, if you consent, your child will receive a free BinaxNOW rapid test for the COVID-19 virus . Collecting a specimen for testing involves using a swab, similar to a Q-Tip, placed inside the tip of the nose. A school staff member who has been trained to use this test will collect the specimen and a trained COVID-19 test administrator will oversee the process. Test results will be made available to the parent/guardian who signs this form below.
