Kindergarten - 5th Grade Enrollment Steps

ENROLLMENT STEPS
 

We strongly encourage all prospective families to attend an AXIS Information Meeting to become more familiar with our school's unique mission and curriculum. Register for an in-person or virtual AXIS information session.

STEP 1: Intent to Enroll Form
An Intent to Enroll Form must be completed for each incoming student, kindergarten through 5th grade.
 
STEP 2: Admission
Beginning in December, AXIS will notify parents/guardians of their student's admission status. ONCE ADMITTED, a link will be provided to begin the next step of Online Enrollment. In order to be enrolled this Online Enrollment (OLR) MUST be completed for each incoming student.
 
STEP 3: Enrollment Paperwork
In December, links to enrollment paperwork will be listed on this site. All accepted students will need to complete this paperwork in order to finalize their enrollment. We ask that families complete and submit all documentation within 14 days of the initial enrollment offer.
 
STEP 4: Completion of Enrollment
Once you have completed the steps above and we have received the necessary forms, AXIS will notify you that the enrollment process is complete.
 
ENROLLMENT PAPERWORK (Grades Kindergarten - 5th Grade)
 
The following records must be submitted within 14 days of an enrollment offer.

  • Proof of Age (such as birth certificate, vaccination record, passport) 

  • Student Vaccination Records or Vaccine Exemption Certification

  • Student Health Entrance Form 2026-2027

  • Home Language Survey

  • AXIS Annual Consents & Acknowledgements 2026-2027

  • Student Records Request Form
    ONLY required for students who are transferring from another K - 5 school.
