Pre-K Enrollment Steps
STEP 1: Fill Out AXIS International Academy Preschool Interest Form
An AXIS Preschool Initial Application must be completed for each incoming Pre-K student.
STEP 2: Attend an Information Meeting
Prospective AXIS International Academy Preschool families are required to attend an AXIS Information Meeting.
Our preschool is intended for students and families who wish to pursue dual language immersion through elementary school. Therefore it is important for families to completely understand the AXIS vision and mission by attending an information session.
STEP 3: Acceptance
In January, links to enrollment paperwork will be listed on this site. All accepted students will need to complete this paperwork in order to finalize their enrollment.
STEP 4: Complete Enrollment Paperwork
Please see details about required preschool paperwork outlined below. All forms must be completed and submitted to AXIS before enrollment is official.
Preschool Enrollment Paperwork
Once your student has been accepted to attend AXIS International Academy Preschool, and you have completed the Online Enrollment (OLR), there are a number of other forms which must be completed to finalize your student's enrollment. Many of these forms are required by state and/or federal laws, therefore no exceptions or omissions can be made.
Required Forms & Documentation
- Student Health Entrance Form 2026-2027
- Home Language Survey
- AXIS Annual Consents & Acknowledgements 2026-2027
- General Health Appraisal Form - This form must be completed and signed by your child's primary healthcare provider (MD, PA, NP) and dated within the last 30 days. Families can also choose to submit a well child visit health summary from an online patient health portal. The form must still be dated within the last calendar year.
- Immunization Records - Required for all students, even immunization exemptions must be documented and on file. See the Colorado Department of Public Health & Environment website for more information.
- Birth Certificate (Proof of Age) - This is required for all students who plan to participate in Universal Preschool Colorado (UPK).
Universal Preschool Colorado
All children in Colorado can register for up to one year of 15 hours of tuition-free, high-quality preschool in their year before kindergarten, as defined by your local school district, through Colorado’s Universal Pre-K (UPK) Program. Families may qualify for more assistance based on a number of qualifying factors. Families must register through UPK following the guidelines laid out on their website.
