Please complete the following form to officially withdraw from AXIS International Academy.

Completion of this form will withdraw your student from AXIS International Academy. Their enrollment seat may be offered to another student in our enrollment pool or on our waitlist.

Please complete one form for each student you plan to withdraw. You must be the student's legal gaurdian to complete this form. If you have joint custody or educational rights of this student, please discuss this option with the other parent before completing a withdrawal.
