By signing this form, I agree to to the following:

I agree that by completing this form, my child has permission to be dismissed from AXIS International Academy to their middle school sibling(s) named below.
Once dissmised from AXIS International Academy, I am aware that AXIS Staff will no longer be responsible for supervising them. I understand that it is my responsibility to make my child aware that, for their safety, they are to walk directly to the established CEC pick up area with their older sibling, and that they are not allowed to loiter around the premises.
I acknowledge that it is my responsibility to notify the AXIS front office when the CEC sibling is absent. When the CEC sibling is absent, I will follow the typical AXIS pick procedures outlined on the AXIS Pick Up and Drop Off page.
I acknowledge that ANY CHANGES from the above permission will be communicated (in writing/email) to the front office. This permission form will remain in effect unless and until you have notified the school in writing that it has been rescinded.
