AXIS Family & Student Handbook Acknowledgement

All AXIS parents and guardians are responsible for reading the contents of the AXIS Family Handbook. This document contains essential information regarding school policies and procedures. You can access an electronic copy of the handbook here.

AXIS Student & Family Handbook*

I affirm that I have received a copy of the 2022-23 AXIS Family & Student Handbook, and have read it. It is my responsibility to refer to the handbook for information concerning school policies and procedures.

Emergency Contacts / Student Pick Up*

I understand that I must list at least two up-to-date emergency contacts for my student who, in case of sickness or emergency, can pick them up within an hour of being notified. I acknowledge that if the school is unable to reach any emergency contact and the student is not picked up within two hours social services may be contacted.

Classroom Directory Consent*

I do not give consent. I understand that this may result in not receiving communication about parent/guardian organized classroom events or out-of-school playdates and parties.

End of Universal Free Lunch*

I understand that federal funding for universal free school lunch has ended. My student is not guaranteed free school lunch daily. AXIS strongly encourages all families to apply for Free/Reduced Lunch. Any families who wish to participate in the school lunch program must follow the instructions on the Nutrition Page (see link above).
